Singapore is facing an unprecedented demographic crisis. In 2025, the nation’s resident total fertility rate (TFR) plunged to a historic low of 0.87, a significant drop from 0.97 the previous year. This trajectory poses profound existential challenges for the city-state, threatening its long-term economic vitality and social fabric. Recognizing the severity of the situation, the government recently established the Marriage and Parenthood (M&P) Reset Workgroup, chaired by Minister Indranee Rajah. This inter-agency body is tasked with developing a whole-of-society approach to support Singaporeans in their marriage and parenthood journeys.
As the workgroup prepares to propose long-term roadmaps and policy enhancements, it is highly likely they will advocate for expanding the scope and availability of fertility counseling, fertility health support, and public education. However, for these initiatives to be truly effective, they must confront a pervasive but often unspoken barrier: the persistent anxiety and fear of Down syndrome among older women undergoing in vitro fertilization (IVF) treatment.
The decision to have children in Singapore is profoundly influenced by a complex interplay of personal aspirations, economic realities, and societal expectations. In recent decades, a significant trend has emerged: an increasing number of individuals and couples are delaying parenthood. While delayed childbearing offers personal and professional advantages, it is linked to a natural decline in female fertility and an increased risk of chromosomal abnormalities. As women age, the risk of aneuploidy rises, and Trisomy 21, or Down syndrome, is one of the most recognized and feared conditions.
In Singapore, this biological reality is exacerbated by specific sociocultural conditions. The nation’s hypercompetitive, Confucian-influenced society places immense emphasis on academic prowess and intellectual achievement. Within this context, parents who are having fewer children feel incentivized to invest heavily in each child to ensure their success. Consequently, the prospect of raising a child with Down syndrome carries a heavy psychological burden. There is a palpable fear of the perceived shame and embarrassment associated with having a disabled child in a society that prizes perfection. Furthermore, parents often harbor feelings of anxiety over the immense financial costs required for special education and the lifelong needs of a child with Down syndrome, viewing it through a pragmatic lens as an investment with minimal "returns."
These profound fears and anxieties create a vulnerability that the IVF industry often exploits. To mitigate the risk of chromosomal abnormalities, prospective parents are increasingly pushed toward Preimplantation Genetic Testing for Aneuploidy (PGT-A). This controversial technique involves biopsying a developing embryo to screen for chromosomal abnormalities before it is transferred to the uterus. The industry heavily markets PGT-A as a definitive solution, a necessary safeguard to ensure a "perfect" baby and improve IVF success rates.
To date, PGT-A has not yet been approved as a mainstream clinical procedure in Singapore. It is still under evaluation by a clinical trial at public hospitals that have so far yielded inconclusive results. To evade restrictive regulations many Singaporean patients travel overseas to access PGT-A at foreign IVF clinics.
However, the reality of PGT-A is far more complex and troubling. The technique is fraught with numerous technical flaws and limitations. It requires an invasive biopsy that carries the risk of damaging the embryo. More alarmingly, PGT-A is prone to misdiagnosis due to the phenomenon of mosaicism—where an embryo contains a mixture of normal and abnormal cells. Because the biopsy only samples a few cells from the outer layer (trophectoderm), it cannot reliably determine the chromosomal makeup of the entire embryo.
Crucially, recent research has illuminated the remarkable biological plasticity of human embryos. Mosaic embryos possess the capacity for "embryonic self-correction," wherein they can normalize their chromosomal makeup as they develop, marginalizing or destroying abnormal cells. Consequently, PGT-A’s rigid binary classification often leads to the tragic discarding of potentially viable, healthy embryos.
The clinical efficacy of PGT-A is also highly contested. Large-scale randomized controlled trials, such as the STAR trial, have failed to demonstrate any significant improvement in ongoing pregnancy rates or cumulative live birth rates with the use of PGT-A. Professional bodies have taken notice. The American Society for Reproductive Medicine (ASRM) and the Society for Assisted Reproductive Technology (SART) have issued committee opinions highlighting the lack of proven benefit for all women. Similarly, the UK's Human Fertilisation and Embryology Authority (HFEA) has assigned a "red traffic light" rating to PGT-A for most fertility patients, indicating that it may reduce the number of embryos available for transfer and potentially decrease the chance of having a baby.
The high error rate and misleading marketing of PGT-A have even sparked significant legal backlash. Multiple class-action lawsuits have been filed against prominent genetic testing companies, alleging consumer fraud, breach of warranty, and deceptive marketing. Plaintiffs argue they were misled about the accuracy and utility of PGT-A, resulting in the devastating loss of potentially healthy embryos and immense emotional and financial distress.
Given these realities, it is imperative that Singapore's expanded fertility counseling and public education directly address Down syndrome anxiety and the misconceptions surrounding PGT-A. Counseling must emphasize a crucial statistical reality: the absolute risk of conceiving a baby with Down syndrome via IVF, even for older women, remains relatively low.
Furthermore, prospective parents must be made well-aware of the various technical flaws and limitations of PGT-A. They need to understand the risks of embryo damage, the high potential for misdiagnosis due to mosaicism, and the reality of embryonic self-correction. The lack of clinical evidence supporting improved live birth rates must be communicated transparently.
Most importantly, public education and counseling must highlight the availability of much cheaper, less invasive, and highly accurate diagnostic alternatives for Down syndrome detection during pregnancy. Non-Invasive Prenatal Testing (NIPT) and advanced ultrasound scans offer robust screening options without the risks associated with embryo biopsy. NIPT, a simple blood test performed during the first trimester, analyzes cell-free fetal DNA in the mother’s bloodstream and boasts high detection rates for common aneuploidies. While NIPT is a screening test that may require diagnostic confirmation, it provides a safe and cost-effective alternative to the expensive and controversial PGT-A procedure.
As Singapore navigates its demographic crisis and seeks to reset societal attitudes toward marriage and parenthood, empowering prospective parents with accurate information is paramount. By addressing the root causes of Down syndrome anxiety and exposing the limitations of aggressive IVF industry marketing, the nation can foster a more supportive, ethical, and evidence-based environment for family formation. Only through transparent counseling and comprehensive public education can we ensure that the pursuit of parenthood does not come at the cost of unnecessary anxiety, financial exploitation, and the tragic loss of viable embryos through PGT-A misdiagnosis.
